How do you correct notes that turn an assumption into a decision?
Identify the passage, distinguish the discussed condition from an approved decision and ask the authorised person to confirm. Do not silently change consequential wording or treat your memory as the sole record.
Preserve a correction trail
- Flag the disputed sentence.
- Provide available contemporaneous notes or your current understanding.
- After confirmation, update status and date and inform people relying on the record.
Correct the level of commitment
Publish next week differs from consider next week if testing is complete. Mark the date unconfirmed before it becomes an external announcement, then record the actual decision. The issue is commitment, not simply tone.

Worked revision: Next week is a candidate release date, subject to completed testing and owner confirmation; corrected today.
